Traditions Hospice

Our team at Traditions Hospice is committed to quality and compassionate care in the final season of life. Care is provided where the patient lives, including but not limited to their own home, a skilled nursing facility, personal care home or retirement community. From the first day of the referral, our Hospice team provides emotional and medical support for the patient and their family. We provide our patients with all related medical equipment, medications and supplies to keep them comfortable at home. Our Physical Therapists educate our patients and their families regarding energy conservation techniques and how to safely care for their loved one in the home environment. Our volunteers provide relief for caregivers, as well as companionship for our patients. Volunteers can also help at nursing homes with special activities, including games, ice cream socials and devotionals.
